Complete the COVID-19 self declaration form

Imperial’s COVID-19 Contact Tracing Hub (CCT Hub) supports staff and students. This is in addition to the NHS Test and Trace service.

This page contains guidance on what to do if you:

  • develop COVID-19 symptoms
  • have received a positive test for COVID-19
  • have been in contact with someone who has symptoms or has tested positive for COVID-19

If you have COVID-19 symptoms or a positive test result, you must report this to the CCT Hub regardless of whether you are studying or working remotely, or have been on or off campus recently. This is important so that we can offer you support, prevent the spread of infection and protect our Imperial community. 

 
 
 
 
 
 

I have COVID-19 symptoms

If you have COVID-19 symptoms, you must:

  1. Self-isolate immediately and remain in your accommodation. If you live in halls of residence, visit the dedicated webpage Self-isolating in halls for College support including food deliveries.  
  2. Report your symptoms and share details of possible close contacts by completing the COVID-19 self declaration form. You can view the information you'll be asked for in the questionnaire.
  3. Book an NHS COVID-19 test as soon as possible by calling 119 or through the  NHS webpage
  4. Inform your programme or course manager. 
  5. Take the COVID-19 test. 

If your test is POSITIVE

  1. Inform the CCT Hub by responding to the email they have sent you. 
  2. Continue to isolate in your accommodation for 10 full days from the day after your symptoms started/start). More information on when you can end self-isolation.

If your test is NEGATIVE

1. Inform the CCT Hub by responding to the email they have sent you.
2. You can leave self-isolation if:

  • You feel well and have a normal temperature.
  • You have not been advised to self-isolate as a contact of a confirmed or suspected case.
  • You have completed any self-isolation period required because you travelled to the UK from a non-exempt country in the past 14 days.

3. Update your COVID-19 status on MyImperial by completing the COVID-19 self declaration form.

I have tested positive for COVID-19

If you have a positive test result, you must:

  1. Self-isolate immediately and remain in your accommodation. If you live in halls of residence, visit the dedicated webpage Self-isolating in halls for College support including food deliveries.
  2. Report your positive test and share details of possible close contacts by completing the COVID-19 self declaration form. You can view the information you'll be asked for in the questionnaire.
  3. Inform your programme or course manager.

College COVID-19 tests

If you were tested via the College COVID-19 testing scheme, your positive test result and contact details will be passed on to Public Health England to allow the NHS Test and Trace contact tracing process to be activated. As COVID-19 is a notifiable disease the College's laboratory has a statutory duty to make this report.

The result is also reported to the CCT Hub to activate the College's own contact tracing service. The Hub will email to prompt you to complete the COVID-19 self declaration form if you have not already done so.

I’ve been identified as a close contact with somebody who has symptoms, but not tested positive yet

If you are contacted by the CCT Hub to advise you that you are a close contact and are therefore at risk of carrying the virus, you must:

  1. Begin a 10 day isolation period in your accommodation. If you live in halls of residence, visit the dedicated webpage Self-isolating in halls for College support including food deliveries.
  2. Record your COVID-19 isolation status on MyImperial by completing the COVID-19 self declaration form.
  3. Inform your programme or course manager.

If the CCT Hub advises you that the contact tested POSITIVE

  1. Complete 10 days self-isolation. The Hub will advise you on when you can resume normal activities.
  2. If you develop symptoms arrange an NHS test and continue to self-isolate until your test result is known.
    If your test is positive inform the Hub via the COVID-19 self declaration form

If the CCT Hub advises you that the contact tested NEGATIVE  

  1. You can resume normal activities unless you are unwell or are in isolation because of some other contact. 
  2. Update your COVID-19 status on MyImperial by completing the COVID-19 self declaration form.
  3. Inform your course or programme manager.

I’ve been identified as a close contact with somebody who has tested positive for COVID-19

  1. You must begin a 10 day isolation period in your accommodation. If you live in halls of residence, visit the dedicated webpage Self-isolating in halls for College support including food deliveries. 
  2. Expect to be contacted by both the CCT Hub covidcthub@imperial.ac.uk and NHS Test and Trace.
  3. Record your COVID-19 isolation status on MyImperial by completing the COVID-19 self declaration form.
  4. Inform your course or programme manager.

If you develop symptoms while you are in self-isolation, you need to arrange an NHS test and continue to self-isolate. Inform the Hub via the COVID-19 self declaration form and update the Hub when you have your test result. 

Whilst self-isolating as a contact, you should not request a COVID-19 test unless you develop symptoms. Even if the test result is negative, you still need to complete your 10 days self-isolation period.
