© 2019 Georg Thieme Verlag KG Stuttgart New York. It looks like treatment of central venous obstruction has become easy since the introduction of iliac stenting procedures. However, this treatment includes a minimum period of anticoagulation and the stent is for life. Furthermore, there are no accepted criteria for evaluation of the results. It is difficult also to establish robust indications of stenting especially since 25 % of the normal population have a diameter reduction > 50 % in the left iliac vein. Another confounding fact is that chronic venous insufficiency (CVI) can be the consequence of multiple simultaneous pathologies, like varicose veins, obstruction and deep venous reflux. Thus, the treatment of one of these may not have a significant clinical effect if the cause is multifactorial. Air Plethysmography (APG) is an objective, non-invasive and patient independent investigation of venous drainage. This includes gravitational filling (venous filling index) and elevation emptying (venous drainage index) all in the same test. This investigation should be performed as a screening test, prior to deep venous stenting and also as a follow-up instrument.

© Springer International Publishing AG, part of Springer Nature 2018. A 67-year old man presented with left leg discomfort and gaiter pruritus which interfered with his retirement lifestyle. His symptoms were worse after prolonged standing and towards the end of the day. Below knee graduated elastic compression (GCS) stockings provided him with some relief. He suffered with poliomyelitis at 12 years old which caused leg muscle wasting. Seven years previously he had his left great saphenous vein (GSV) stripped to the knee with multiple phlebectomies. This alleviated similar symptoms but resulted in persistent ankle edema.

ObjectiveMedical compression stockings are a standard, non-invasive treatment option for all venous and lymphatic diseases. The aim of this consensus document is to provide up-to-date recommendations and evidence grading on the indications for treatment, based on evidence accumulated during the past decade, under the auspices of the International Compression Club.MethodsA systematic literature review was conducted and, using PRISMA guidelines, 51 relevant publications were selected for an evidence-based analysis of an initial 2407 unrefined results. Key search terms included: ‘acute', CEAP', ‘chronic', ‘compression stockings', ‘compression therapy', ‘lymph', ‘lymphatic disease', ‘vein' and ‘venous disease'. Evidence extracted from the publications was graded initially by the panel members individually and then refined at the consensus meeting.ResultsBased on the current evidence, 25 recommendations for chronic and acute venous disorders were made. Of these, 24 recommendations were graded as: Grade 1A (n = 4), 1B (n = 13), 1C (n = 2), 2B (n = 4) and 2C (n = 1). The panel members found moderately robust evidence for medical compression stockings in patients with venous symptoms and prevention and treatment of venous oedema. Robust evidence was found for prevention and treatment of venous leg ulcers. Recommendations for stocking-use after great saphenous vein interventions were limited to the first post-interventional week. No randomised clinical trials are available that document a prophylactic effect of medical compression stockings on the progression of chronic venous disease (CVD). In acute deep vein thrombosis, immediate compression is recommended to reduce pain and swelling. © 2018 The Authors Objectives: Hydrostatic compression (HC) occurring during body immersion is considered beneficial for venous return from the lower limbs. No study has evaluated the effects of HC on the veins of the lower limbs using duplex ultrasound (DU). The purpose of this study was to use DU to evaluate the morphology and flow of the leg veins during immersion. Design: The femoral and great saphenous veins were evaluated before and during immersion in a specifically built pool, in normal and varicose limbs. Results: HC reduces vein size in both normal and varicose limbs. During immersion spontaneous flow increases whereas when present, blood reflux decreases. Discussion: This pilot, proof of concept study has demonstrated the feasibility of DU investigation of leg veins during immersion. Larger series of underwater DU evaluations of normal and varicose legs are necessary to quantify and better explain the effects of HC on the veins of the lower limb.
