Psychomotor research is essential for aptitude-based selection of surgical trainees and sound surgical practice. Two microprocessor-controlled psychomotor testers were developed to evaluate psychomotor skills related to endoscopic surgery. Dundee Endoscopic Psychomotor Tester (DEPT) measures single-handed performance in an endoscopic environment and therefore it can be used to evaluate differing abilities between the right and left hand. Advanced Dundee Endoscopic Psychomotor Tester (ADEPT) measures two-handed performance, and consequently it can be used to assess coordinated bimanual endoscopic manipulations. Psychomotor testers provide real-time objective scoring systems that have several aspects of face validity to real endoscopic environment. Studies on medical students have confirmed that objective evaluation of task performance in an endoscopic field is feasible and have documented differences in psychomotor abilities between subjects. © 1997 by W. Lymphoma of the appendix is exceptionally rare and the management of this condition is still controversial. A multimodality approach (surgery, radiation therapy, chemotherapy) is the optimal treatment. The Authors report a case of lymphoma of the appendix treated by right hemicolectomy and postoperative multiagent chemotherapy.

BACKGROUND: There is little reported information on psychomotor performance in relation to minimal access surgery (MAS). METHODS: A microprocessor-controlled endoscopic psychomotor tester (the Dundee Endoscopic Psychomotor Tester-DEPT) has been developed to evaluate psychomotor aspects of MAS. Experiments were conducted on 20 medical undergraduates to evaluate accuracy and reliability of the tester. RESULTS: The study demonstrated a significant difference between subjects (p < 0.01). It also identified three individuals who enacted 16, 22, and 40 errors while the majority (85%) sustained less errors with a median of 4.5. CONCLUSIONS: DEPT provides a standard, reproducible, objective real-time scoring system. It identifies individuals who cannot adjust to endoscopic viewing and therefore manipulate from endoscopic images.

INTRODUCTION Advanced endoscopie procedures have become a popular alternative to many major surgical operations. The aim of the study is to investigate the effect of experience and training of the surgeon on endoscopie surgical performance. METHODS The standard task was tying a surgeon's knot with 2/0 silk around a cylinder in an endoscopie trainer. Ten surgeons with varying experience in endoscopie surgery participated in the study. The first group (five surgeons) performed only basic laparoscopic operations and attended advanced laboratory courses to learn intracorporial endoscopie knotting while the second group had experience in advanced laparoscopic procedures. The same task was performed six times with the end of the telescope at distances of 50, 75, 100, 125 and 150 mm from the suture. The effect of the optical axis of the telescope (0, 30 and 45 degrees) and manipulation angles of the instruments (30, 60 and 90 degrees) were controlled. The execution time of each knot was recorded as a measure of task efficiency. Each knot was analysed by distraction tensiometry. The number of slipped/broken knots and the force required to break or reverse slip each knot were used as a task quality measure. MannWhitney Latest and Kruskal-Wallis one way AXOVA were used for analysis. RESULTS One hundred and sixty-seven out of 1350 knots were slipped in the group II whereas 245 knots were slipped in the group I. Logistic regression showed a significant difference between the two groups with P< 0.0001. Group II had a shorter execution time (mean 84 s) than Group I (mean 93 s) with P< 0.0001. Group II also had a higher force (mean 22.3 Newton) than Group I (mean 21.1 Newton) with P< 0.001. There was a significant difference between surgeons in the number of slipped/broken knots, the execution time and the force within each group (P< 0.0001).
