Infection with HIV results in the dissemination of the virus to a number of target compartments which may act as sanctuary sites for the virus. Sanctuary sites for HIV may be divided into cellular and anatomical sites. Cellular sites for HIV include both productively and latently infected CD4+ lymphocytes, macrophages and dendritic cells while anatomical sites include blood, lymph nodes, central nervous system (CNS), genital tract, spleen and lung. Some of these sites may be inaccessible to some classes of antiretroviral drugs e.g. some protease inhibitors penetrate poorly into the CNS and genital tract. Mathematical models have been used to predict the importance of sanctuary sites and trafficking of virus between compartments on the pathogenesis of HIV infection. This review will discuss recent understanding of the role of sanctuary sites in the persistence of HIV in the host.

Infection and gene expression by the human T lymphotropic virus type I (HTLV-I) in vivo have been thought to be confined to CD4+ T lymphocytes. We show here that, in natural HTLV-I infection, a significant proportion of CD8+ T lymphocytes are infected by HTLV-I. Interestingly, HTLV-I-specific but not Epstein-Barr virus-specific CD8+ T lymphocytes were shown to be infected. Furthermore, HTLV-I protein expression in naturally infected CD8+ T lymphocytes renders them susceptible to fratricide mediated by autologous HTLV-I-specific CD8+ T lymphocytes. Fratricide among virus-specific CTLs could impair the immune control of HTLV-I and possibly other lymphotropic viruses.

Phylogenetic analysis of nucleotide sequences of gag and env genes of type 1 human immunodeficiency virus (HIV-1) variants isolated from individuals infected through sexual intercourse or nosocomially (by injections with nonsterile syringes) showed that 5 of 27 (18.5%) isolated strains were recombinants. Two viruses found in the Russian Far East had gagAenvE genotype, three other recombinants had envG genotype; gag gene of one isolate belonged to subtype A and gag genes of two others belonged to subtype D. Detection of new recombinant variants in addition to the A/B recombinant described previously shows that these viruses can contribute to the HIV-1 genetic variability in Russia.
