With the advent of antibiotics syphilis and its complications appear to have been declining as an important cause of cardiovascular disease. We describe a patient with an unusual aorto-pulmonary communication secondary to syphilitic aortitis. The case illustrates the difficulty in defining the anatomy of this rare association preoperatively. The reason for the rarity of this manifestation is discussed and finally doubt is cast upon the assumption that cardiovascular syphilis is of only historical interest.

BACKGROUND AND AIMS OF THE STUDY: Carcinoid syndrome originates from carcinoid tumors localized in the gastrointestinal system, pancreas, biliary vessels, bronchi, ovaries and testes. The condition is characterized by flushing, telangiectasias, diarrhea, bronchoconstriction and cardiac involvement, while cardiac lesions may occur in 50% of patients. This study aimed to investigate-non-invasively-the frequency, severity and evolution of carcinoid heart involvement. METHODS AND RESULTS: Among 87 patients with carcinoid syndrome undergoing routine echocardiography, 39 (45%) had cardiac involvement; 36 of these had thickened, shortened, immobile tricuspid leaflets. Tricuspid regurgitation was present in all 36, it was moderate or severe in 22. Pressure half-time rose to 117 +/- 35 ms, implying mild tricuspid stenosis. The pulmonary valve was also thickened, retracted and immobile with stenotic appearance in 19 patients; regurgitation was present in 16, and mild in 11. Nine patients had pericardial effusion. Of 39 patients with cardiac lesions, 18 had at least one follow up study within 16 +/- 7 months; in 15 cases the valve lesions deteriorated. Tricuspid regurgitation became more severe in six and pressure half-time rose from 107 +/- 46 to 132 +/- 50 ms. Pulmonary regurgitation deteriorated in two, while three patients had new pulmonary valve involvement. The peak gradient across the pulmonary valve increased from 18 +/- 7 to 25 +/- 9 mmHg. Twenty-two patients without heart involvement in the first examination had a follow up study (19 +/- 5 months) during which seven developed valvular lesions, with four moderate and three mild tricuspid regurgitation; mild stenosis was present in all cases. We describe a young woman with homozygous familial hypercholesterolaemia complicated by aortic stenosis, supravalvular aortic atheroma and coronary artery disease. The association of these disorders is common in homozygotes and usually requires surgical intervention with a high operative risk. Our patient underwent successfully widening of the aortic root, aortic valve replacement and aortocoronary bypass grafting, but 2 out of 4 similarly treated patients died.

Introduction: Measurement of cardiothoracic ratio (CTR) on chest X-ray is routinely used as an index of cardiomegaly. Echocardiography is currently extensively used for assessment of cardiac anatomy and chamber dimensions. However, CTR remains the first choice measurement for an overall impression of cardiac size in clinical practice because of its simplicity and wide understanding. Methods: We studied 73 consecutive patients referred for echocardiographic assessment of the cardiac anatomy following the detection of increased cardiothoracic ratio during routine postero-anterior chest X- ray. The cardiac chambers dimensions, the ventricular septum and the posterior left ventricular wall thickness and the presence of pericardial effusion were evaluated. Results: Echocardiography was normal in 30 patients (41%) while 7 patients (9%) had pericardial effusion with normal cardiac dimensions. In 25 patients (34%) dilatation of left ventricle with or without left atrial enlargement was observed. Eleven patients (16%) had left ventricular hypertrophy with or without left atrial enlargement. CTR was best related with left atrial size (p<0.01) as well as with the end-systolic and end-diastolic left ventricular diameter (p<0.05). CTR greater than 0.57 was always related with pathologic findings by echocardiography. Conclusions: We conclude that although measurement of CTR on chest X-ray provides a quick estimation of cardiac size, it does not differentiate normal from abnormal heart. Echocardiography provides accurate measurements of individual cardiac chamber dimensions and readily detects the presence of pericardial fluid.

OBJECTIVE: To test whether the silence of painless myocardial ischemia is caused by abnormal handling by the central nervous system of afferent messages from the heart. DESIGN: Nonrandomized study. SETTING: A tertiary referral center (postgraduate medical school). PATIENTS: 2 matched groups of nondiabetic patients with coronary artery disease. Group A consisted of nine patients with reproducible stress-induced angina; group B consisted of nine patients with reproducible stress-induced myocardial ischemia but no angina. INTERVENTIONS: Intravenous placebo infusion and low-dose (5 and 10 micrograms/ kg per minute) and high-dose (20 to 35 micrograms/kg per minute) dobutamine infusions. MEASUREMENTS: Positron emission tomography was used to measure regional cerebral blood flow changes as an index of neuronal activation during painful and silent myocardial ischemia induced by intravenous dobutamine. RESULTS: Regional cerebral blood flow changes during myocardial ischemia were compared with those during baseline conditions and during placebo infusion. During myocardial ischemia, regional cerebral blood flow increased bilaterally in the thalami and prefrontal, basal frontal, and ventral cingulate corticles in patients in group A. Both thalami were activated in group B, but cortical activation was limited to the right frontal region. A formal comparison of groups A and B showed significant differences (P < 0.01) in activation of the basal frontal cortex, ventral cingulate cortex, and left temporal pole. In both groups, thalamic regional cerebral blood flow remained increased after the symptoms and signs of ischemia had ceased. CONCLUSIONS: Bilateral activation of the thalamus can be shown in both angina and silent ischemia; thus, peripheral nerve dysfunction cannot completely explain silent ischemia.
