We have previously described an investigational ultrasound imaging device which is based on multiple piezoelectric elements and a DEC computer system. The basic technology has evolved into a clinically applicable tool. As part of the evaluation of the system, a series of comparisons was made between direct and ultrasonic measurements of multiple arterial diameters. The overall result showed a median difference of 4%. It should be noted that a typical approach to comparison (as reported in the literature) is to compare areas which is less critical. While the accurate measurement is vital, the clinician also demands information as to the nature of the disease process affecting the underlying wall. This is where ultrasound can eclipse other imaging techniques. The standard approach to tissue characterization has been to relate changes in tissue structure to changes in the amplitude of the A-mode signal. This has been shown to be unreliable. We have obtained more satisfactory results by combining amplitude with frequency and phase information. This depends on the use of advanced frequency tracking based on linear estimation. This approach has led to very satisfactory differentiation in two dimensional models (B-mode). The power of our system in clinical terms has been to use this 2-D information to reconstruct differentiated three dimensional models. The voxel space software allows the clinician to individually examine and manipulate the different tissue structures identified in this way. The geometric relationship between different types of extracted tissue can be retained throughout complex 3-D data manipulation. The clinical importance of this is to enable accurate assessment of atheromatous plaque both before and after angioplasty.
