Magnetic navigation has been established as an alternative to conventional, manual catheter navigation for invasive electrophysiology interventions about a decade ago. Besides the obvious advantage of radiation protection for the operator who is positioned remotely from the patient, there are additional benefits of steering the tip of a very floppy catheter. This manuscript reviews the published evidence from simple arrhythmias in patients with normal cardiac anatomy to the most complex congenital heart disease. This progress was made possible by the introduction of improved catheters and most importantly irrigated-tip electrodes.

The population of adults with congenital heart disease is growing. Most children with congenital heart disease now survive into adulthood thanks to innovations in cardiac surgery that allow repair or palliation of many anatomical defects. However, arrhythmia and sudden death remain important causes of late morbidity and mortality. As the population of adult congenital heart disease (ACHD) ages there is an increasing burden of arrhythmia requiring specialist care. Rhythm disturbances may be the first presenting symptom and herald a need for surgical or transcatheter reintervention in which case treatment should be directed at the underlying cardiovascular hemodynamics. In this case electrophysiological intervention is a secondary concern. Arrhythmia and sudden cardiac death may, alternatively occur in the absence of a target hemodynamic lesion. Arrhythmia mechanisms vary according to the exact underlying anatomic congenital defect and method and timing of surgical repair. Whilst the arrhythmia can relate to underlying structural heart disease, such as Wolff-Parkinson-White syndrome associated with Ebstein's anomaly, it often relates to surgically acquired scars combined with chamber enlargement as a consequence of abnormal pressure and volume loads. In congenital heart disease, atrial arrhythmias frequently emanate from the right atrium or right ventricle and are not generally confined to the left atrium as with atrial fibrillation triggered by pulmonary vein muscle bundles. ACHD patients with atrial arrhythmia are at particular risk of tachycardia induced cardiomyopathy and existing hemodynamic lesions such as valvular regurgitation may be exacerbated by arrhythmia. There is also overlap in clinical presentations, such that ACHD patients presenting with atrial arrhythmia are at higher risk of ventricular arrhythmia [1]. PURPOSE: Cardiovascular magnetic resonance assessment of adults late after an atrial redirection operation for transposition is demanding and time consuming. We hypothesised that the relatively fast and standardised 3-dimensional time-resolved contrast-enhanced magnetic resonance angiography, or dynamic angiography, would be valuable in the periodic follow-up of these patients. METHODS: We investigated prospectively 36 adults with transposition using dynamic angiography, comparing our results against a comprehensive but non-contrast cardiovascular magnetic resonance protocol. We acquired 6 dynamic angiographic datasets after injection of contrast. The primary aim was to detect significant obstruction of the pathways for venous flow. RESULTS: In 4 patients (11%), we found evidence of moderate-to-severe, and thus clinically important, obstruction of systemic venous channels on standard cardiovascular magnetic resonance. All these patients were correctly identified by dynamic angiography. In 4 additional patients, we found mild and haemodynamically insignificant obstructions in the systemic venous channels. Of the 8 (22%) patients with any obstruction, 6 were detected by angiography. There were no false positives reported, giving sensitivity of 75% and specificity of 100%, a positive predictive value of 100%, and negative predictive value of 93%. In 1 patient, there was a moderate obstruction of the pulmonary venous compartment which was not readily seen by dynamic angiography. CONCLUSIONS: 3-dimensional dynamic angiography is a useful method for detecting anatomically moderate-to-severe, but not mild, obstructions in the systemic venous channels following Mustard repair for transposition. This technique can be used as a single imaging method and/or as complimentary to standard two dimensional cardiovascular magnetic resonance techniques for detection of clinically important obstructions in the systemic venous channels.

Cardiovascular magnetic resonance imaging (CMR) has become a cornerstone in the diagnosis and management of congenital heart disease and selected paediatric cardiology patients. This article reviews the relevance of late gadolinium enhancement (LGE) CMR in both congenital and acquired heart diseases. Commonly, adult LGE CMR is with a view to understanding myocardial viability in older ischaemic heart disease patients, where the extent, location, pattern and transmurality of LGE relate to longer term outcomes. However, LGE findings also contribute to diagnosing non-ischaemic cardiomyopathies and their potential aetiology. New uses also include using LGE CMR as an outcome measure in the assessment of interventions including pharmacotherapies. With time there is emerging data regarding a potential role for LGE CMR in risk stratification for arrhythmia and sudden cardiac death. There is therefore understandable interest in its relevance from clinicians involved in assessing and caring for cardiovascular disease in the young, whether congenital or acquired and early reports in these settings are discussed. After outlining aspects concerning principle, practice and pitfalls of the technique, the described clinical experience of late gadolinium enhancement will be discussed. Robust research will be required to fully understand where LGE CMR will fit in with routine clinical paediatric cardiology and adult congenital practice but it seems likely to have a unique role to improve diagnosis and management strategies, determine response to therapies, and assess long term prognosis, not only in ischaemic heart disease patients, but in these cardiac patients too. © 2009 Elsevier Ireland Ltd.
