Cadaveric median nerves were divided and repaired with interrupted epineural sutures, as in normal clinical practice, using three different gauges of nylon monofilament material. Tensile testing of the nerve repairs showed that, of these three gauges, 9/0 (0.3 metric) sutures were preferable: gap formation at the repair site occurred at the highest force. Finer gauge (10/0) sutures snapped at a lower force, while thicker (8/0) sutures were less consistent and pulled out of the nerve tissue.

A biomechanical study in vitro has evaluated a new modification of the core and peripheral suture technique for flexor tendon repair. Groups of repairs were conducted in cadaver tendons, using a core suture alone, a core suture with a simple running surface suture and a new modification involving a 'Halsted' horizontal mattress technique for the peripheral stitch. The Halsted modification increased the load at which a visible gap formed by 93%, the load at which a 2 mm gap formed by 77%, and the maximum strength by 89%. This increase was due to the technique; it did not depend on the suture material used. The bulk of the tendon repair was not significantly greater with the Halsted modification.

1. X-ray, cine and video recordings were made of the movement of radio-opaque markers injected into the musculo-tendinous junctions of biceps brachii muscle. 2. In strong isometric contractions, the distal tendon of the long head of biceps lengthened by about 2% of its estimated rest length. 3. During voluntary isotonic elbow flexion-extension movements at frequencies up to 5.5 Hz there was no detectable phase shift between intramuscular and joint displacements. 4. In the fastest alternating movements (5.5-6.7 Hz) small phase advances developed in the muscle. 5. We conclude that human tendons do stretch during muscle contraction, but not enough to cause intramuscular phase reversals in rapid unloaded movements. This in turn means that muscle spindles shorten and lengthen virtually in phase with joint movements under most conditions.

The continuation of an unacceptable failure rate with tendon repair or grafting procedures, largely due to adhesions, suggested that an artificial flexor tendon could be an attractive alternative. A literature search found no published data of the mechanical properties of fresh human finger flexor tendons, so a study of the strength and extensibility of 153 tendons was carried out. The bone insertion strength of twenty middle finger tendons was also examined. The results showed that an artificial tendon should have a strength of approximately 1500N, and that it should extend 13% at that load, an elongation of 26mm for a tendon 200mm long. The insertion strength was less than a half of the tendon strength. This data will allow an artificial flexor tendon to be designed with sufficient strength and the correct elastic properties to allow its function to integrate reliably with natural tendons in adjacent fingers.
