Background Anti-neutrophil cytoplasm antibody (ANCA) associated vasculitis with renal involvement requires treatment with potentially toxic drugs to reduce morbidity and mortality, and there is a major challenge to determine clinical and histological features predictive of renal prognosis. The aim of our study was to evaluate the use of the 2010 international histological classification for ANCA-associated glomerulonephritis (AAGN) as a predictor of renal outcome when used in conjunction with other prognostic factors.Methods One hundred and four patients with AAGN treated at our centre were included: 23 were classified as focal, 26 as crescentic, 48 as mixed and 7 as sclerotic. Renal outcomes were based on estimated glomerular filtration rate (eGFR) at 1 and 5 years, and on renal survival.Results By univariate analysis, patients in the focal class had the best renal outcome, those in the sclerotic class the worst outcome, and those in the mixed and crescentic classes had intermediate renal survival. There was no significant difference in outcome between the mixed and crescentic classes. In multivariate models, histological class did not improve model fit or associate with renal outcome after adjusting for established prognostic factors. Lower percentage of normal glomeruli, greater degree of tubular atrophy (TA), MPO-ANCA positivity, increasing age and lower starting eGFR, all correlated with poorer renal outcomes.Conclusions We conclude that, in our cohort of patients, the international histological classification is predictive of renal outcome in AAGN, but did not appear to be additionally informative over other established prognostic factors in multivariate analysis. However, it may be of value to combine the current histological classification with other established parameters, such as TA and percentage normal glomeruli.

Background. The ATP-sensitive P2X7 receptor (P2X7R) hasbeen shown to contribute to renal injury in nephrotoxic nephritis,a rodent model of acute glomerulonephritis, and in unilateral uretericobstruction (UUO), a rodent model of chronic interstitialinflammation and fibrosis. Renal tubular cells, endothelial cellsand macrophages also express the closely related P2X4 receptor(P2X4R), which is chromosomally co-located with P2X7R andhas 40% homology; it is also pro-inflammatory and has beenshown to interact with P2X7R to modulate its pro-apoptotic andpro-inflammatory effects. Therefore, we chose to explore the functionof P2X4R in the UUO model of renal injury using knockoutmice. We hypothesized that UUO-induced tubulointerstitialdamage and fibrosis would also be attenuated in P2X4R−/− mice.Method. P2X4R−/− and wild-type (WT) mice were subjectedto either UUO or sham operation. Kidney samples taken onDays 7 and 14 were evaluated for renal inflammation and fi-brosis, and expression of pro-fibrotic factors.Results. To our surprise, the obstructed kidney in P2X4R−/−mice showed more severe renal injury, more collagen deposition( picrosirius red staining, increase of 53%; P < 0.05) andmore type I collagen staining (increase of 107%; P < 0.01), aswell as increased mRNA for TGF-β (increase of 102%, P <0.0005) and CTGF (increase of 157%; P < 0.05) by Day 14,compared with the UUO WT mice.Conclusion. These findings showed that lack of P2X4Rexpression leads to increased renal fibrosis, and increasedexpression of TGF-β and CTGF in the UUO model.

A 72-year-old male patient with known diffuse cutaneous systemic sclerosis (SSc) presented with severe haemoptysis and blood and protein in the urine. In light of his known interstitial lung disease, he had been repeatedly treated for recurrent community-acquired pneumonia. Immunological testing demonstrated a strongly positive perinuclear antineutrophil cytoplasm antibody with a high titre antimyeloperoxidase antibody. The patient was diagnosed with pulmonary-renal syndrome as a consequence of antineutrophil cytoplasm antibody-associated vasculitis. He started immediate plasmapheresis in combination with methylprednisolone, followed by cyclophosphamide and rituximab, with good clinical outcome.

Rationale: Hypoxia followed by reoxygenation promotes inflammation by activating NF-κB transcription factors in endothelial cells (EC). This process involves modification of the signalling intermediary TRAF6 with polyubiquitin chains. Thus cellular mechanisms that suppress TRAF6 ubiquitination are potential therapeutic targets to reduce inflammation in hypoxic tissues. Objective: In this study, we tested the hypothesis that endothelial activation in response to hypoxia-reoxygenation can be influenced by Cezanne, a deubiquitinating enzyme that cleaves ubiquitin from specific modified proteins. Methods and Results: Studies of cultured endothelial cells (EC) demonstrated that hypoxia (1% oxygen) induced Cezanne via p38 MAP kinase-dependent transcriptional and post-transcriptional mechanisms. Hypoxia-reoxygenation had minimal effects on pro-inflammatory signalling in unmanipulated EC but significantly enhanced Lys-63 polyubiquitination of TRAF6, activation of NF-κB and expression of inflammatory genes following silencing of Cezanne. Thus although hypoxia primed cells for inflammatory activation it simultaneously induced Cezanne which impeded signalling to NF-κB by suppressing TRAF6 ubiquitination. Similarly, ischemia induced Cezanne in the murine kidney in vascular EC, glomerular EC, podocytes and epithelial cells, and genetic deletion of Cezanne enhanced renal inflammation and injury in murine kidneys exposed to ischemia followed by reperfusion. Conclusions: We conclude that inflammatory responses to ischemia are controlled by a balance between ubiquitination and deubiquitination and that Cezanne is a key regulator of this process. Our observations have important implications for therapeutic targeting of inflammation and injury during ischemia-reperfusion.
