Human mesenchymal stem cells (MSC) can be isolated from adult bone marrow and either extensively expanded without differentiation or induced to differentiate into multiple mesenchymal lineages. The aim of this study was to investigate the characteristics of MSC during the first trimester of human life. First trimester fetal blood (n=15; median gest age: 10: wks), fetal liver (n=2,10° and 14° wks) and fetal bone marrow (n=2, 11 2 and 14 wks) samples were collected and nucleated cells cultured at 10 5/ml in DMEM with 10% FBS.Single fibroblastic-like colonies were stained and scored: mean number (± SEM) of colonies: 8.2 ±0.6, 11.3± 2.0 and 12.6± 3.6/10 nucleated cells from fetal blood, liver and bone marrow , respectively. Fibroblast-like cell colonies from first trimester fetal blood (n=3; median gest age: 93 wks), liver (n=l; 14 wks) and bone marrow (n=l, II2 wks) were plucked after 2 passages in culture.Single colony- derived cells could be expanded in vitro for at least 20 passages. Fetal blood, liver and bone marrow derived single colonies showed mean cumulative population doublings of 50.3 ±4.5, 52.2 ±5.3 and 49.5 ±3.5, respectively. In their undifferentiated state, clonal adherent cells from fetal blood, liver and bone marrow did not express CD45, CD34, CD14, CD68, vWF, HLA-DR antigens while they produced prolyl-4-hydroxylase, a-smooth muscle actin, fibronectin, laminin, vimentin and reacted positively with mesenchymal cell specific antibodies (SH2, SH3, SH4). When individual fetal blood, liver and bone marrow adherent cell colonies were cultured in hydrocortisone, isobutylmethylxanthine and indomethacin for 2 weeks the cells underwent adipogenic differentiation, displaying large vacuoles which were oil red O positive; after 2 weeks of culture in the presence of osteogenic medium (dexamethasone, ascorbic acid, β-glycerophosphate) calcium deposition was detected by silver nitrate and Alizarin red stain
