Six people with multiple health problems living in an extra care housing scheme for older people with vision impairment agreed to take part in a telecare trial. An average of 14.8 sensors was installed in each of the flats. The monitoring period began in January 2006 and lasted for 10 months. The data acquired by the sensors installed in each flat was transmitted from the home unit to a central computer. Four interviews were conducted with each participant. Halfway through the study we created a number of case studies of sensor activity at the time of known events in the lives of the participants, together with an attempt to interpret these patterns of activity with the benefit of available contextual information. Although our investigation showed that sensors are capable of identifying some changes in daily routines at the time of important events, the interpretation of such changes requires a large amount of contextual information and the involvement of participants themselves. Various technicaland operational difficulties will need to be resolved before it will be possible to use lifestyle monitoring predictively.

Governments and care providers are increasingly turning towards information and communications technology (ICT) as a way of supporting and enhancing existing health care services. Policies to develop various forms of telehealthcare are being put in place across Europe. In the UK, there are ambitious plans to roll out a home telemonitoring service to everybody who needs it by 2011.

This paper discusses the relationship between different forms of design and technological innovation and their ability to support older people in their own homes. It considers (1) innovation in the design and construction of new housing, notably 'lifetime homes' and 'open building' systems; (2) the introduction of electronically enhanced assistive technology; and (3) telecare. Each of these is examined in terms of its potential benefits for meeting the housing needs of older people and its limitations. The paper proposes that these innovations are likely to have a variable effect on the possible housing pathways of older people, depending on the way they are combined. It also outlines the policy and market influences that may stimulate their adoption, concluding that design and technology innovations must be matched by new care delivery models.

We studied the introduction of a telecare and rehabilitation scheme in north-west Surrey. It was decided not to create a new team to provide the new services, but to involve established teams and individuals. The introduction of telecare therefore added to the roles and responsibilities of these teams. This staffing policy helped to establish awareness of the project within the local care system and to support its subsequent deployment. An education and training unit was established to demonstrate what the technology could do and to act as a focus for training health and social care professionals. The study suggested that for telecare to achieve its full potential, pilot projects must be designed to be evaluated, and more attention must be paid to the degree of integration with the care system as a whole.
