The most frequently used de?nition of an intermediate (IL) or borderline lesion is that of a lesion causing a 40% to 60% diameter stenosis on angiography. This de?nition stems from experimental models showing that a 75% lumen area stenosis corresponding to a 50% lumen diameter stenosis signi?cantly impairs maximal hyperemic ?ow. Nevertheless, visual estimation, which is most often used to evaluate stenosis severity with angiography, is beset by a high inter-observer variability and inaccuracy when compared to physiologic assessment. In one study, only half of the physiologically nonsigni?cant lesions were characterized as such by three independent operators. Presumably for this reason, the most recent American College of Cardiology (ACC) and European Society of Cardiology (ESC) guidelines have broadened the angiographic de?nition of intermediate coronary stenoses up to 70% luminal narrowing (1, 2).
