Summary

Dr Nandor Marczin is Clinical Senior Lecturer at Imperial College and honorary consultant in Adult Cardiothoracic Anaesthesia at the Royal Brompton and Harefield NHS Foundation Trust. Dr Marczin is program director of clinical academic training in anaesthesia. He is also co-ordinator of the Perioperative Medicine module of BSc in Surgery and Anaesthesia

 

He graduated from Pecs Medical University in 1987 in Hungary and started a residency in anaesthesiology and intensive care at the Pecs University Hospital. He then spent a fruitful postdoctoral and junior faculty period at the Department of Pharmacology at the Medical College of Georgia in the USA working in the field of vascular biology and pharmacology. After completing his residency training in Hungary, he took up further clinician scientist posts at the Royal Brompton and Harefield Hospitals and at Imperial College London. As a recipient of the prestigious Medical Research Council Clinician Scientists Fellowship, he directed his interest in novel diagnostics and treatment of patients undergoing routine and more specialised cardiothoracic surgery including heart and lung transplantation.

 

Dr Marczin’s research is focussing on molecular, cellular and physiological mechanisms of perioperative inflammation and metabolic derangements with a strong emphasis on real time molecular diagnostics at the bedside and novel therapies that combine molecular approaches with sophisticated technological platforms (Figure 1).

The main experimental platforms include  

  • cellular models such as isolated leukocytes, whole blood, cultured vascular and alveolar epithelial cells (to pursue basic mechanistic questions),
  • animal models of acute lung injury and systemic inflammation (to establish physiological relevance or proof of principle therapeutic effects; in collaboration with Prof Takata)
  • patients undergoing routine or high risk cardiac surgery, oesophagectomy  and cardiothoracic organ transplantation (Figure 2)

Current projects include

 

  • mechanisms and influence of ageing and frailty in cardiac surgery
  • inflammatory and metabolic aspects of oesophagectomies
  • Application of exhaled breath molecular analysis in perioperative medicine (exhaled nitric oxide, carbon monoxide, ethylene, volatile organic compounds using chemiluminescence, SIFT-MS and laser methods
  • Inhaled nitric oxide therapy in cardiac surgery including assist device implantation
  • Management of multiorgan donor and donor heart and lung assessment
  • Ex vivo evaluation and reconditioning of injured donor lungs and hearts
  • Minimally invasive lung transplantation
  • Mechanisms and monitoring post lung transplant infections and rejection
