In order to assess the incidence of valvular regurgitation in dilated cardiomyopathy, 26 patients, aged 32 to 61 years, were prospectively studied by two-dimensional echocardiography and color doppler flow imaging. Patients were in New York Heart Association functional class III or IV. All patients underwent coronary angiography which excluded coronary artery disease. Contrast ventriculography showed a dilated and hypocontractile left ventricle and right ventricular endomyocardial biopsy excluded specific heart muscle disease. Regurgitant flows were observed as a mosaic pattern and were semiquantitatively classified as mild, moderate or severe according to the width and depth of the regurgitant jets. Twenty-six (100%) patients had mitral regurgitation, 9 (34,6%) had aortic regurgitation, 12 (46.2%) had tricuspid regurgitation and 11 (42.3%) had pulmonary regurgitation. Multivalvular regurgitation was observed in 77% of patients occurring across two, three, or all four valves in 30.8%; 30.8% and 15.4% of patients, respectively. Two dimensional echocardiography color flow imaging shows a high prevalence of multivalvular regurgitation in patients with dilated cardiomyopathy.

Pacing wire thrombosis may be invisible echocardiographically while producing severe symptoms. We report a patient whose pacing wire thrombosis presented as a pyrexial illness 5 months after its insertion. Treatment with tissue plasminogen activator provoked cardiovascular collapse; therefore, we recommend that such thrombosis be treated surgically.

Mitral regurgitation in hypertrophic cardiopathy has been considered to occur as a result of systolic anterior motion (SAM) of the mitral valve. To further assess the relation between mitral regurgitation and SAM, color Doppler echocardigraphy was performed in 78 consecutive patients with hypertrophic cardiomyopathy. Twenty-six patients had complete SAM of the mitral valve with septal contact, 24 had incomplete SAM without septal contact and 28 did not have SAM. Mitral regurgitation was observed in 24 of the 26 patients with complete SAM (92%), in 8 of the 24 patients with incomplete SAM (33%), and in 6 of the 28 patients without SAM (21%). A mosaic pattern in the left ventricular outflow tract was seen in all patients with complete SAM, in 4 with incomplete SAM and in 2 without SAM, and this was readily distinguishable from the mosaic flow of mitral regurgitation. The onset of mitral regurgitation was 94 ± 29 ms after the R wave, whereas the onset of SAM was 120 ± 20 ms after the R wave. In 84% of occurrences of mitral regurgitation, the regurgitant signal was detected before the onset of SAM. Although complete SAM was associated with mitral regurgitation, the appearance of mitral regurgitation prior to the onset of SAM suggests that it may not be caused by SAM.
