Tyrer P, Tyrer S, 1974, School refusal, truancy, and adult neurotic illness, Psychological Medicine, Vol: 4, Pages: 416-421, ISSN: 0033-2917

SYSNOPSIS
Two hundred and forty adult psychiatric patients, comprising 60 phobic, 60 anxious, and 120 depressed patients, were interviewed about problems of school attendance during their childhood. Using a specially designed questionnaire, non-attendance due to school refusal was swparated from that due to truancy. A control population of 120 orthopaedic and dental patients, matched for age and sex with the psychiatric sample, was also interviewed. It was found that school refusal had occurred significantly more often among the psychiatric patients than among the controls (P < 0·05). The incidence of truancy was similar in both groups. Both school refusal and truancy had occurred more frequently in female than in male patients. The results suggest that, contrary to some studies, there is a link between childhood and adult neurotic disorders and that some children who present with problems of school refusal are at risk in adult life.

Tyrer P, Candy J, Kelly D, 1973, Phenelzine in phobic anxiety: a controlled trial, Psychological Medicine, Vol: 3, Pages: 120-124, ISSN: 0033-2917

Synopsis
A double-blind trial of phenelzine versus placebo in psychiatric outpatients with long-standing phobic anxiety is described. Forty patients with agoraphobia or social phobias entered the trial and 32 of these completed treatment with either phenelzine or placebo (with appropriate dietary restrictions) in flexible dosage for two months. Fourteen pairs of patients were prospectively matched, and analysis of the score differences between these pairs showed a significant improvement for patients on phenelzine compared with placebo on overall assessment (P<0·01) and secondary phobias (P<0·05). These differences were not shown at assessment after one month's treatment, but became marked after two months. The improvement on phenelzine, which was maintained at follow-up, was not related to initial depressive symptoms in the patients. It is suggested that phenelzine is an effective agent in the treatment of phobic anxiety but that treatment needs to be continued for at least two months before full benefit is achieved.

No unequivocal central effects were found with either +/--propranolol (120 mg) or +/--sotalol (240 mg) in acute dosage in normal subjects. Subjective feelings of drowsiness and muzziness were found with sotalol and both sotalol and propranolol caused subjects to feel more troubled. These changes were not accompanied by physiological or behavioural evidence of sedation. Adequate beta-adrenoceptor blockade was achieved as measured by a significant fall in pulse-rate on both active drugs but no other significant peripheral physiological changes occurred.
