In this study we analyzed the acromial spurs of 15 patients with impingement syndrome undergoing open rotator cuff repair. Mineral apposition analysis and quantitative cytochemical techniques for glucose-6-phosphate dehydrogenase (G6PD) activity (pentose phosphate pathway), alkaline phosphatase (ALP) activity (osteoblast activity), and tartrate-resistant acid phosphatase (TRAP) activity (osteoclast phenotype) were used to examine the distribution and level of activity of selected marker enzymes within the acromial spur insertion into the coracoacromial ligament in order to establish whether local behavior of bone cells is consistent with the proposed secondary development of the acromial spur. Our results indicate that G6PD and ALP activity was higher in osteoblasts on the inferior surface compared with the superior surface of the acromial spur in all patients (P <.001). This area correlated to the most intense area of mineral apposition shown by dual tetracycline labeling. TRAP activity revealed a heterogeneous distribution within the samples. A greater G6PD activity per cell (mean increase of 87%) was seen at the tip compared with that in post- and pre-tip zones within the coronal plane (P <.0002). The qualitative and quantitative enzyme analyses show that the acromial insertion of the coracoacromial ligament is actively involved in bone turnover. The spatial distribution patterns of metabolically active bone-forming osteoblastic cells compared with a heterogeneous distribution of TRAP-positive osteoclasts provide evidence of bone remodeling consistent with the morphologic contours of the acromial enthesis. The sites of oxytetracycline labeling appear to correlate with the sites of high ALP and G6PD activity, which supports the concept of spur formation being a secondary phenomenon in the presence of established rotator cuff tears.

Severity of glenohumeral arthropathy following shoulder instability is often graded radiologically on the basis of osteophyte size according to Samilson and Prieto. Intra- and inter-observer reliability of this popular grading system was determined using standard radiographs of 20 arthritic cadaveric humeri. Using two observers on two occasions, reliability of the grading system using the antero-posterior radiograph varied between fair and poor (kappa coefficients 0.07-0.33). This study shows that the Samilson and Prieto grading of glenohumeral arthropathy is unreliable and, in clinical use, when magnification and rotation is less easy to control, it is likely to be even more so. Another validated classification system must therefore be found.

The strength component of the Constant score has been criticized for its lack of a precisely defined measurement method. A series of experiments was performed to compare three different methods in normal and pathologic shoulders with the use of a standard test position. These were (1) the Isobex isometric dynamometer, (2) Constant's unsecured spring balance, and (3) a new modification in which the spring balance is fixed at one end and the reading is taken after 5 seconds of maximum effort. The results suggest that this simple modification with a low-cost spring balance can give similar values to those from the Isobex. The need for precision of terms and a definition of the method is discussed, and recommendations for the standardization of the many variables in making this measurement are made.

Variation in acromial morphology has been associated with the pathogenesis of subacromial impingement syndrome and rotator cuff disease. However, the use of a multitude of different descriptive terminology has lead to confusion in the literature concerning acromial architecture. This review is aimed to show the discrepancy in the literature of acromial morphology and advise suitable terminology to prevent confusion in the future.

We report the 20-year results of Bonnin's modification of the Bristow-Latarjet procedure in 14 patients operated on by one surgeon. All but one patient had had traumatic dislocations. At review, the Rowe scores were excellent in five, good in eight and fair in one. The functional outcome was satisfactory, with a mean Constant-Murley score of 80 points (68 to 95), but 12 patients had restriction of external rotation (86%). There were radiological degenerative changes in ten shoulders (71%): six in Samilson grade I, one in grade II, and three in grade III. Isometric power was considerably reduced in patients with grade-III degenerative change. This operation provides good long-term shoulder stability, but the high incidence of radiological degenerative change is a cause for concern.

Dr. Jules Emile Péan is widely credited with having performed the first total shoulder replacement March 11, 1893, at the Hôpital International in Paris. However, in his original report Péan refers to the work of Themistocles Gluck as being the inspiration for his shoulder prosthesis, a fact understated if not completely overlooked during the last hundred years. This article therefore attempts to reappraise the relative contributions of these two pioneering surgeons to shoulder arthroplasty.

Sickle-cell disease (SCD) is probably the commonest cause of avascular necrosis worldwide, and its prevalence appears to be rising in developed countries. Avascular necrosis of the humeral head is a common complication but has not been previously studied in detail. We have reviewed 138 patients with SCD for clinical, radiological and functional abnormalities of the shoulder, using a radiological classification designed for avascular necrosis of the shoulder. Radiographic lesions, frequently bilateral, were found in 28% and only 53% of patients had normal shoulder function. The management of this relatively common complication is difficult. Joint replacement is likely to fail and early diagnosis is important.

The aim of this investigation was to determine whether some metabolic defect might be related to the propensity of osteoporotic femoral necks to fracture acutely. To this end, the activities of two dehydrogenases of the glycolytic Embden-Meyerhof pathway, two of the pentose phosphate pathway, two mitochondrial enzymes, and alkaline phosphatase were measured in the cortical and in the trabecular osteoblasts. Comparison was made with such activities in iliac crest biopsies from patients with these fractures and from the equivalent femoral and iliac crest samples from patients with osteoarthritis of the hip, in biopsies from the iliac crests from seven patients with no bony abnormality, and in specimens from the fracture site of six traumatic fractures. The results showed a highly significant decrease in the activities of the two enzymes of the pentose phosphate pathway (p less than 0.001) in the cortical, but not in the trabecular, osteoblasts in the osteoporotic fractures. This could not be attributed to the trauma of acute fracture since it was not found in traumatic fractures. Other experimental evidence has indicated that a relationship may occur between depressed activity of these enzymes and a retardation of bone formation.

On the basis of studies with bone-seeking isotopes, it is generally believed that the metabolic activity of osteoblasts of the trabecular bone is greater than that of the osteoblasts of the cortex. This implies that the oxidative activity, which provides the energy for biosyntheses, will also be greater in the former than in the latter. In the present study, direct measurement of the activities of representative enzymes of the major oxidative pathways, as well as of alkaline phosphatase, showed that the reverse pertained. However, owing to the greater cellularity of trabecular bone, the activity per unit mass of bone may be higher in the trabecular bone.
