BACKGROUND: Epidemiological evidence from observational studies has suggested that blood levels and dietary intake of selenium of adults with asthma are lower than those of controls. The only previous trial of selenium supplementation in adults with asthma found no objective evidence of benefit but involved only 24 participants. METHODS: A randomised, double blind, placebo-controlled trial of selenium supplementation was performed in adults with asthma in London, UK, the majority of whom (75%) reported inhaled steroid use at baseline. 197 participants were randomised to receive either a high-selenium yeast preparation (100 microg daily, n=99) or placebo (yeast only, n=98) for 24 weeks. The primary outcome was asthma-related quality of life (QoL) score. Secondary outcomes included lung function, asthma symptom scores, peak flow and bronchodilator usage. Linear regression was used to analyse the change in outcome between the two treatment arms by ""intention to treat"". RESULTS: There was a 48% increase in plasma selenium between baseline and end of trial in the active treatment group but no change in the placebo group. While the QoL score improved more in the active treatment group than in the placebo group, the difference in change in score between the two groups was not significant (-0.05 (95% CI -0.19 to 0.09); p=0.47). Selenium supplementation was not associated with any significant improvement in secondary outcomes compared with placebo. CONCLUSIONS: Selenium supplementation had no clinical benefit in adults with asthma, the majority of whom were taking inhaled steroids.

An algorithm is presented for calculating concordance-discordance totals in a time of order N log N , where N is the number of observations, using a balanced binary search tree. These totals can be used to calculate jackknife estimates and confidence limits in the same time order for a very wide range of rank statistics, including Kendall's tau, Somers' D, Harrell's c, the area under the receiver operating characteristic (ROC) curve, the Gini coefficient, and the parameters underlying the sign and rank-sum tests. A Stata package is introduced for calculating confidence intervals for these rank statistics using this algorithm, which has been implemented in the Mata compilable matrix programming language supplied with Stata.

multproc carries out multiple-test procedures, taking as input a list of p-valuesand an uncorrected critical p-value, and calculating a corrected overall critical pvaluefor rejection of null hypotheses. These procedures define a conÞdence regionfor a set-valued parameter, namely the set of null hypotheses that are true. Theyaim to control either the family-wise error rate (FWER) or the false discoveryrate (FDR) at a level no greater than the uncorrected critical p-value. smileplotcalls multproc and then creates a smile plot, with data points corresponding toestimated parameters, the p-values (on a reverse log scale) on the y-axis, and theparameter estimates (or another variable) on the x-axis. There are y-axis referencelines at the uncorrected and corrected overall critical p-values. The reference linefor the corrected overall critical p-value, known as the parapet line, is an informalÒupper confidence limitÓ for the set of null hypotheses that are true and defines aboundary between data mining and data dredging. A smile plot summarizes a setof multiple analyses just as a Cochrane forest plot summarizes a meta-analysis. Copyright 2003 by Stata Corporation.
