The last decade has seen an unprecedented development in imaging technologies that are able to noninvasively characterize cardiovascular conditions. At the forefront of these modalities have been cardiovascular magnetic resonance (CMR) and multidetector computed tomography angiography (MDCTA). Linked to advances in software and hardware, current CMR scanners are able to provide a comprehensive evaluation of the cardiac patient with a range of information on anatomy, function, perfusion, viability, tissue characterization, flow patterns, as well as coronary and vascular angiography. MDCTA is able to provide accurate characterization of coronary luminal stenosis and plaque burden in a short scan, frequently avoiding the need for subsequent x-ray angiography with its attendant risks and complications - particularly in the intermediate risk group. Developments in echocardiography have enabled better tissue characterization, real-time three-dimensional visualization, and, using specific contrast agents, assessment of myocardial perfusion. Tissue Doppler techniques have facilitated better understanding of myocardial mechanics - particularly useful, eg, in assessment of dyssynchrony. In this review article we provide a brief overview of how these techniques work and discuss current and future applications. We highlight the strengths, as well as limitations, of these imaging modalities, and discuss when to consider these tests in clinical practice. Copyright © 2007 LLS SAS. All rights reserved.

A 55-year-old man developed an acute myocardial infarction and was bitten by a snake with haemotoxic venom prior to any treatment being given. We discuss our treatment of myocardial infarction in a patient who had been unintentionally anticoagulated.

With an escalating health burden from coronary artery disease (CAD), both as a public health and fiscal issue, evaluation of patients with (CAD) remains an important challenge. With advances in therapy our management options have increased, coinciding with a shift away from treatment alone, to disease prevention and accurate risk stratification. Imaging techniques play an important role in guiding management. Recent advances in cardiovascular magnetic resonance (CMR) enable a detailed non-invasive assessment of patients with a combination of anatomical and functional data to assess the impact of CAD in individual patients. The range of sequences means that myocardial perfusion, viability, coronary angiography and plaque characterization are available in a single study. Outcome data have helped with disease prognostication. In this review, we discuss current and future applications of CMR and consider which patients would benefit from a scan.

Hypertrophic cardiomyopathy (HCM) is an inherited cardiac disorder characterized by unexplained myocardial hypertrophy. The condition is associated with sudden cardiac death and is therefore often diagnosed postmortem, especially in the young and in competitive athletes. For this reason, intense research focuses on developing strategies to minimize this tragic consequence. Cardiovascular magnetic resonance (CMR) is a novel imaging modality that provides high-resolution images in an infinite number of planes with additional sequences that allows for tissue characterization and quantification of flow. The most exciting development is the application of late gadolinium-enhanced (LGE) imaging, which allows for in vivo detection of myocardial fibrosis. This review summarizes the current applications of CMR in HCM and also speculates on future applications, particularly the potential for risk stratification using LGE-CMR.

AIMS: Previous studies have raised concerns about high levels of anxiety and depression in implanted cardioverter-defibrillator patients, and suggested that adverse psychological outcomes have been related to delivered therapy, age, and gender. This study aimed to assess the prevalence of anxiety and depression and to analyse quality-of-life in a New Zealand patient group. METHODS AND RESULTS: We questioned 46 ICD and 49 pacemaker patients regarding device and treatment satisfaction, depression, anxiety (Hospital Anxiety and Depression Scale), and quality-of-life (SF 36). The prevalence of clinical depression and anxiety in the ICD group was 7 and 13%, respectively, and did not differ from the pacemaker group. ICD patients mean anxiety and depression scores did not differ from the pacemaker group, although more ICD patients had subclinical levels of anxiety. Quality-of-life scores were normal for all ICD patients with respect to both mental and physical component scores, and not different from the pacemaker group. Anxiety, depression, and quality-of-life scores were unrelated to time from implantation, delivered therapy, age, or gender. Overall, 93% of the ICD patients thought their device was worthwhile. CONCLUSION: We found a lower than expected level of anxiety and depression in ICD patients, and suggest that this may be due in part to the small team approach adopted locally in the follow-up of this patient group.
