Myocardial calcification is a rare finding usually detected by computerised tomographic (CT) scanning. It is often missed and, when found, is often misdiagnosed. The addition of magnetic resonance imaging (MRI) to our investigative armamentarium enables correct diagnosis and appropriate management.

Cardiovascular magnetic resonance (CMR) has established itself as probably the single best way of phenotyping the failing heart. It is the accepted gold standard for measuring cardiac function, volumes, and mass, but within the same scan session additional techniques are available for greater definition. Tissue characterization with the contrast agent gadolinium is well validated and allows the precise visualization and quantification of myocardial infarction. This can be used for viability assessment and to determine heart failure etiology. Dobutamine stress CMR and CMR perfusion hold advantages over conventional techniques. The new frontiers of CMR in heart failure hold the promise of unique insights quantifying myocardial iron, nonischemic fibrosis, microvascular perfusion, plaque characterization, and CMR-targeted intervention. The development and validation of these techniques represent major research challenges for the future. From a clinical perspective, an equal challenge is in increasing the availability of the modality for patients and physicians.

Chronic heart failure is a common disorder placing significant burdens on patients and health-care services. Noninvasive imaging plays a central role in accurate diagnosis, determination of etiology and prognosis, and in monitoring therapy. Advances in technology mean cardiovascular magnetic resonance (CMR) imaging has established itself as both a valuable clinical and research tool in this arena. Not only is CMR the new gold standard for accurate and reproducible assessment of ventricular volumes and mass, but by using gadolinium contrast, underlying pathology can often be determined. In ischemic cardiomyopathy a 'one stop' assessment can be made of function, perfusion and mass. Continuing advances such as myocardial tagging and the increasing availability of CMR mean that it will become an increasingly important and useful tool for clinicians looking after patients with cardiomyopathy and heart failure.

Inspite of important advances in therapeutic interventions for patients with left ventricular (LV) dysfunction and congestive heart failure (CHF), the prevalence of these conditions continues to rise. Accurate and reproducible assessment of ventricular function and mass is essential for diagnosis of ventricular dysfunction and assessment of prognosis, as well as for guiding therapeutic interventions and monitoring therapy. Cardiovascular magnetic resonance (CMR) is ideally suited for fulfilling this task. We review here how CMR can be applied in CHF incorporating techniques to assess strain, scarring and fibrotic change as well as viability and perfusion, both in clinical practice and in research.
