Objective: To assess regional changes in ultrasound-derived indices of blood flow in the dominant human follicle after the plasma LH surge. Design: A cross-sectional, prospective study. Setting: Reproductive medicine unit at a university. Patient(s): Women attending an assisted conception clinic to determine the appropriate time to transfer previously frozen embryos during a natural cycle. Intervention(s): Transvaginal ultrasonography with color Doppler imaging and pulsed Doppler spectral analysis was used to obtain indices of blood flow and velocity from vessels in the base, lateral part, and apex of the dominant follicle on days 10-12 (from day 1 of menses) and after the LH surge, but before rupture. Immunoassays were used to measure the blood concentrations of LH twice daily (at 8-10 A.M. and 4-6 P.M.) from cycle day 10. Main Outcome Measure(s): The pulsatility index (PI), resistance index (RI), peak systolic velocity (PSV), and time-averaged maximum velocity (TAMXV) in the uterine arteries and three regions of the dominant follicle (apical, lateral, and basal parts); follicular volume; the day and time of the onset of the LH surge (defined as first concentration of LH >22 U/L) and the times of each scan. Result(s): Twenty-two women (aged 28-39 years) were studied and seven were scanned on days 10-12. A retrospective examination of the data from the remainder showed that eight were scanned <20 hours after onset of the LH surge and seven were scanned >20 hours after the onset of the LH surge. There was a significant increase in follicular volume after the LH surge. The PI was similar in vessels from the base (0.86 ± 0.11; mean ± SEM), lateral part (0.72 ± 0.51) and apex (0.67 ± 0.09) at cycle days 10-12 and then gradually decreased in the apex. Transvaginal ultrasonography has the potential to become integral to the management of endometrial pathology for women of all ages. In this review we outline the reproducibility of measuring endometrial thickness and examine the data that suggest that a single measurement of endometrial thickness may be used to avoid an invasive biopsy in many symptomatic postmenopausal women. An endometrial thickness of <5.0 mm is highly unlikely to contain significant pathology. The actions of hormone replacement therapy are described, and the effects of tamoxifen on the endometrium and their ultrasound characteristics are also outlined. A thick cystic endometrium in a woman taking tamoxifen has a 50% chance of being a polyp and is associated with hyperplasia and cancer. The use of color Doppler with reference to the detection of endometrial pathology and the impact of hydrosonography for the diagnosis of polyps is discussed. Finally, a number of simple clinical guidelines are suggested. © 1995 Lippincott-Raven Publishers, Philadelphia.

OBJECTIVE: To determine the feasibility of a randomised trial of ovarian cancer screening among women attending a breast cancer screening centre. DESIGN: Randomised controlled trial of ovarian cancer screening using transvaginal ultrasonography as a primary screening test and colour Doppler imaging as a secondary screening test in the screened group and no intervention in the control group. SETTING: Reading breast cancer screening centre (United Kingdom). SUBJECTS: 8678 women, without a bilateral oophorectomy or hysterectomy, aged between 50 and 64 attending for NHS breast cancer screening between September 1989 and February 1993. MAIN OUTCOME MEASURES: Uptake of ovarian cancer screening among eligible women and the screening false positive rate (considered as the referral rate to a gynaecologist for surgical intervention). RESULTS: 82% (7124/8678) of eligible women agreed to join the trial and were randomly allocated in equal numbers to each arm of the trial. 3280 women had an initial scan. The false positive rate after ultrasonography alone was 2.9% (86/2952), but this dropped to 0.5% after colour Doppler as a secondary screening test. One stage I primary ovarian cancer was detected at screening in a 58 year old women. CONCLUSIONS: A randomised trial of ovarian cancer screening using ultrasonography and colour Doppler imaging is feasible and acceptable among women attending a breast cancer screening centre. The results indicate that the expected odds of being affected given a positive result in the general population would be about 1:12. A full randomised trial of ovarian cancer screening with mortality as the end point is needed to assess whether screening reduces mortality from this disease. A multicentre European trial is currently in progress.
