The objective of this study was to assess the accuracy of three-dimensional (3D) reconstructions obtained in a clinical setting using a freehand 3D ultrasound imaging system based on an electromagnetic position and orientation measurement device. An ultrasound phantom comprising a 4×2 matrix of non-magnetic pins fixed 25 mm apart in plastic was immersed in 9% glycerol solution and scanned five times using a 3D ultrasound imaging system on a couch used for clinical scanning. Inter-pin distances were then calculated from the 3D reconstructions. 140 inter-pin distance measurements were computed. The root-mean-square error over all five scans was 1.01 mm, with a mean absolute percentage error of 1.85±1.22% (Mean±Standard deviation), and a range of 0.05-3.43%. The results of this phantom study show that accurate measurement of distances from 3D reconstructions is achievable in a clinical environment using a 3D ultrasound imaging system which incorporates an electromagnetic tracking device.

The objective of this study was to develop accurate, robust, automatic methods for quantifying carotid disease using measures derived from three-dimensional (3D) ultrasound reconstructions. A surface model of the carotid vessel wall and atherosclerotic disease was reconstructed from transverse colour Duplex images together with 3D position and orientation data for each slice. The resulting geometric model was used to estimate a series of new measures: in particular, plaque volume, volumetric stenosis (vessel volume/lumen volume), and transverse area stenosis as a function of length along the vessel is computed and compared with standard two-dimensional (2D) B-mode and velocity measures of stenosis. Efficient computer algorithms to compute the various 3D plaque measures have been implemented which highlight some of the inadequacies of conventional, 2D measures of carotid disease. In conclusion, 3D measures of carotid plaques potentially provide a more reliable and accurate index of severity of disease than standard 2D measures.
