The techniques that have been used to characterize the biomechanical behavior of the knee, cruciate ligaments, and cruciate ligament replacements differ, making comparisons between studies difficult or, at times, impossible. Therefore, it is important to standardize the testing protocols and techniques that describe the biomechanical behavior of the knee and cruciate ligaments. This will allow investigators to express opinions with respect to the interpretation of data, rather than based on differences between testing techniques. Standardized techniques are proposed to locate the origins of the tibial and femoral coordinate systems, and thus, allow comparisons of knee kinematics (e.g., displacements and rotations) between investigations. Standard techniques that can be used to measure the load-displacement behavior of the knee are described, and important considerations that should be appreciated with respect to preparing and testing of the joint are summarized. It is important to evaluate the single cycle load-to-failure characteristics and the cyclic loading response of an anterior cruciate ligament graft, and techniques to evaluate cruciate ligament graft fixation are proposed. The strengths of different models to characterize the biomechanical behavior of the knee are reviewed.

After clinical failure of ulnar component fixation by olecranon fracture occurred, this study examined different component designs to find the strongest type. Components requiring square cuts or rounded cuts to prepare the proximal ulna with short or long stems of polyethylene or metal were implanted in paired cadaver bones and tested to failure. Loads were applied through the articular surface in an anterior-posterior direction, causing bending fracture of the olecranon. The main findings were a higher strength if the bone was prepared to take a rounded rather than a square-shaped prosthesis body, a slightly higher strength with a metal component compared with a similarly shaped polyethylene component, and slightly less strength if a longer stem was used. We recommend the use of a polyethylene component with a rounded shape and short stem, because it will be thinner, entailing less bone excavation. Additionally, it is less costly than a metal-backed design.

Experiments were performed to evaluate biomechanical aspects of the performance of a "deep-biting peripheral suture" for flexor tendon repair, either when used alone or with a square or modified Kessler core stitch, and the technique was compared to the Kleinert repair. Tests included progressively increasing cyclic loads, force to pull the repair into the A2 pulley, and ultimate failure strength. 50% of the Kleinert repairs failed under 30 N cyclic loading, while 100% of the DBPS plus Kessler core stitch repairs survived. There was no discernable difference in gliding function or repair bulk between these sutures, but ultimate strength increased significantly with the DBPS repairs. We concluded that the DBPS plus Kessler-type core stitch will survive active mobilization better than the Kleinert method.

The purpose of this study was to review the results of ACL reconstruction using a patellar tendon graft placed 'over the top' plus a Macintosh lateral tenodesis, examining changes in knee laxity and functional status with increasing time. There were 74 patients operated on over an 11 year period, and divided into four groups for analysis according to postoperative time. There was a significant and progressive increase in side-to-side laxity difference with time, although functional status did not change significantly, indicating a lack of correlation between objective clinical tests and subjective findings. The highest Lysholm, Tegner and IKDC scores were at 4-5 years after operation, when 60% of patients were at their pre-injury level of sports activity. However, there was always a very significant difference between actual and desired Tegner activity levels for the group as a whole. While there was a significant correlation between degenerative changes and the time between injury and reconstruction, there was no correlation with postoperative time: this provides evidence that ACL reconstruction can protect the knee from later degeneration. © 1996.
