Across her clinical, education, leadership and research roles, Hadeel is passionate about bridging research and practice to deliver meaningful improvements in patient care.
Short summary: During this webinar Hadeel will share findings from her PhD research exploring how older people with polypharmacy self-manage their medicines across the hospital-to-home transition, with a particular focus on a qualitative study examining patients’ experiences, behaviours and medication-related interactions around discharge. The findings highlight how medication self-management can remain largely invisible within discharge planning and demonstrate how individual, contextual, and system-level factors influence patients’ preparedness to manage their medicines at home. Hadeel will also discuss how these insights informed the development of a person-centred approach to identifying and supporting medication self-management needs during transitions of care.
