AB - Exposure to dust mites in early infancy, or even in utero, is important for primary sensitisation. Reducing exposure offers a potential opportunity for primary prevention of atopy and asthma, but it is essential to implement measures which can achieve and maintain low allergen environment. Manchester Asthma and Allergy Study follows the development of asthma and atopy in a cohort of at risk infants who are randomly allocated to dust mite allergen avoidance or normal regime. Avoidance measures comprise: I) Before 16th week of pregnancy: a) mite allergen-proof bedding for parental bed b) high filtration vacuum cleaner c) bed linen hot washed weekly II) At the 36th week of pregnancy a) vinyl flooring in infants' bedroom b) new cot and carrycot mattress c) Acarosan in the areas of home with Der p 1 levels >2 μg/g d) bed linen hot washed weekly e) soft toys washable in hot cycle. Dust samples from the parental mattress (PM) and bedroom floor (BF), nursery floor (CF) and infants mattress (CM) were collected and Der p 1 levels determined by monoclonal-antibody based ELISA. Der p 1 levels (total allergen recovered - ng; GM and 95% CI) during pregnancy and in early life in 50 active and 50 control family homes on PM and BF are listed in the table. There was also a significant difference between the groups in total recovered allergen in nursery room [active vs control (ng; GM and 95% CI): CM - 7 (4-15) vs 23 (11-48), p=0.03; CF - 2 (0.8-3) vs 40 (21-80), p<0.0001].
